Forward with Dementia is inviting ALL Canadians to take part in a new research study!

Participation involves completing a survey about your knowledge of and attitudes toward dementia and memory loss. We are particularly interested in hearing from people who are members of diverse cultural and linguistic communities.

This survey will take 15-20 minutes to complete. It is available in English, French, Traditional Chinese, Simplified Chinese, Punjabi, Spanish, Italian, Arabic and Tagalog. After participating in this survey, you may enter in a draw for a chance to win 1 of 5, $50 gift cards!

Forward with Dementia aims to address stigma and raise awareness about how to live well with dementia. The website: www.forwardwithdementia.ca, includes stories and resources developed together with people living with dementia, family care partners, and health and social care providers.
