How to Make a Referral to CLBC
- Complete a CLBC Referral Form
The referral form can be filled out by a support worker, family member, caregiver, or the individual themselves.

Include documentation confirming a diagnosis of a developmental disability (e.g., Autism Spectrum Disorder) before age 18.
Psychological assessments or medical records can help support eligibility. - Submit the Completed Form
Submit the referral form and all supporting documents to the CLBC office in the region where the individual currently lives.
