How to Make a Referral to CLBC

  1. Complete a CLBC Referral Form
    The referral form can be filled out by a support worker, family member, caregiver, or the individual themselves.

  2. Gather Supporting Documents
    Include documentation confirming a diagnosis of a developmental disability (e.g., Autism Spectrum Disorder) before age 18.
    Psychological assessments or medical records can help support eligibility.
  3. Submit the Completed Form
    Submit the referral form and all supporting documents to the CLBC office in the region where the individual currently lives.

  4. Wait for CLBC Contact
    A CLBC intake worker will review the application and follow up with next steps, which may include a formal assessment.
