Michelle creates a home where choice, respect, and belonging come first. In a short time as a Home Sharing Provider, she has shown exceptional commitment to person led support, helping the individual she supports build confidence, life skills, and meaningful connections. She encourages exploration, new experiences, and active participation in community life. Together, they have enjoyed many outings and activities that bring joy and a sense of belonging. Michelle understands that independence grows through shared experiences and relationships. Her thoughtful support helps build both daily living skills and lasting connections with friends, family, and the wider community.

Melissa Hyland

Inspiration Award Recipient

For more than twenty years, Melissa has created a home filled with care, patience, and strength. She supports each person to build confidence, learn new skills, and express themselves with understanding and respect. Her belief that everyone deserves to be heard and valued is at the heart of her work. Melissa listens deeply and helps those she supports take part in daily life, from managing health and routines to joining community activities. She has guided people through recovery, helped them rebuild trust, and celebrated every step forward. Proud of her Mohawk Indigenous background, she honours the cultural identities of those she supports, including Indigenous people and new Canadians, helping them stay connected to who they are. Through compassion, consistency, and advocacy, Melissa shows what it means to turn care into lifelong connection and healing.

For nearly thirty years, this Home Sharing Provider has created a home where people feel safe, respected, and part of a family. She always listens and ensures that each person’s voice guides their choices and daily routines. She supports the individuals she cares for to build skills in cooking, budgeting, and self care, and to take part in community life. She celebrates every achievement and helps maintain strong family connections that bring comfort and belonging. Her steady presence and person led approach show how commitment and compassion can transform lives and strengthen the meaning of home sharing.

Home Sharing Provider of the Year Recipient

Jana received four nominations that speak to her deep compassion and lasting impact on the people she supports. Over seventeen years, she has created a welcoming home where everyone feels respected, valued, and included. Jana helps people build confidence and independence by encouraging their choices, supporting daily routines, and guiding them through challenges with patience and care. She is known for her strong advocacy, creativity, and willingness to learn new approaches to meet individual needs. Jana’s home is a place of growth and belonging, where people feel safe to express themselves and stay connected long after they move on. Her commitment, kindness, and consistency have inspired everyone around her and show what true person led home sharing can achieve.

While she entered this role without formal experience, her deep passion for giving back to her community has allowed her to create spaces where people feel safe, valued, and understood. By providing a home where others can truly be themselves, she offers more than just shelter, she offers a sense of security, belonging, and emotional support for those who need it most. Whether through one-on-one interactions or broader community initiatives, she consistently involves individuals in decision-making, ensuring they feel heard, respected, and empowered to take charge of their own lives.  

 

Her focus on fostering a positive life balance demonstrates her understanding that mental, emotional, and physical well-being are all intertwined. Through her work, she’s not merely providing a place to stay or temporary assistance; she’s actively helping people build stable, sustainable lives. She works on providing them with the tools, resources, and guidance to thrive, recognizing that long-term success is rooted in holistic support.  

 

As she continues to grow in her role, her unique perspective, as someone who has learned through personal experience and compassion rather than formal training, sets her apart. Her leadership, grounded in empathy and understanding, has the potential to inspire others in similar roles. By demonstrating the power of tailoring support around the individual’s wants and needs, she shows that true impact comes from building a safety net of health, comfort, and trusts that allows people to flourish.  

 

Through Crystal’s dedication, the individual’s life stabilized and enriched. Her care extended far beyond the day to day; she rebuilt family connections, encouraged new experiences, and created a home filled with support and joy. Crystal’s compassionate approach exemplifies the profound impact of creating a nurturing, balanced environment for healing and growth.

Anne Bazell

2024 Long-Term Service Award Recipient

For years, Anne Bazell quietly carried out some of the most demanding care work in home sharing, supporting two individuals with profound needs.  Anne’s tireless dedication to these two people is amazing and she always did it with a smile on her face. These were two individuals who were hard to place and who were hard to get respite for.  

  

But to Anne, they were simply people who deserved dignity, love, and joy, just like anyone else. Anne offered both individuals not just support, but a sense of home, belonging, and consistency. She did it without complaint, even when  systems were limited.    

  

Anne gave her heart to those who needed it most, and even when no one was watching, she showed up with love, patience, and an unwavering smile. 

Rhianna Millman

2024 Inspiration Award Recipient

Rhianna and her husband Paul opened their home to a young Indigenous man navigating complex medical needs. During a hospital stay, caregivers witnessed firsthand how cultural bias impacted his care. Decisions were made based not on medical needs, but on assumptions about his culture.   

  

Recognizing the injustice, Rhianna took action. She brought the issue forward, and her courage sparked a broader conversation within the health care system. Her advocacy reached the Health Caucus leaders, who are now working with the Health Authority’s Board and Senior Executive Team to address systemic racism and improve care for all.  

  

Her efforts didn’t stop there. ICS is deeply grateful for Rhianna’s continued work to challenge discrimination in health care, both against Indigenous peoples and people with disabilities.  

  

At home, Rhianna and Paul continue to lead with compassion. They support each individual not just with daily routines, but with deep respect and encouragement for their independence. They’ve helped individuals find jobs, build schedules, and navigate public transit so they can meet friends and participate fully in the community.  

  

One individual in their care once stayed silent during planning meetings, allowing others to speak on their behalf. With Rhianna’s gentle encouragement and belief in their abilities, that same person now leads their own meetings and confidently makes decisions about their life.  

  

Quiet support leading to bold self-determination, is the kind of impact Rhianna and Paul make every day. 

 

Judy Yacoboski

2024 HSP of the Year Award Recipient

Judy has spent years standing beside the Disability Community as a quiet force for change. As a long-time advocate and home sharing provider, she supports two young adults in a way that is both deeply respectful and forward-thinking.  

  

What makes Judy stand out is her unwavering belief in people. She sees each person for who they are and meets them where they’re at, with kindness, curiosity, and zero judgment. Her approach is rooted in person-first, strengths-based values, and it shows in every part of her work.  

  

One of the individuals Judy supports once hesitated to connect with others or step outside their comfort zone. But through Judy’s steady mentorship and encouragement, that relationship has blossomed into something beautiful. Today, the two of them are often off on adventures, rock-hunting, hiking forest trails, getting their hands messy with pottery, or simply chasing a new idea that came up in their person-centered planning sessions.  

  

Judy’s support doesn’t just change daily life. It reshapes futures. The individual now has a community around them that truly sees and celebrates who they are. Their dreams are nurtured. Their ideas are heard, even the wobbly ones. And when things don’t go perfectly, Judy is there with quiet guidance and a sense of calm that builds trust and confidence.  

  

The individuals she supports are young (early 20s), and both strive for independence. Living in the detached spaces allow them to learn skills related to independence, including cooking, cleaning, budgeting, scheduling, etc., while having the quite, respectful support of Judy when they need it. Our home sharing team has said, on more than one occasion, that we need to duplicate Judy not only for the support she provides folks, but for the “tiny home” model that is incredibly effective for a lot of individuals.  

  

Judy reminds us that true support is not just about being there, it’s about believing in someone so fully that they begin to believe in themselves.
